This is not the V code table that you see in the official coding guidelines for coding and reporting.  This is a table that breaks down each distinct V code or category of V codes and lists what terms to use to index it in the ICD-9-CM book, whether or not it can or must be first-listed or second listed, a brief discussion on some appropriate uses for the code, a brief discussion on some inappropriate uses for the code, and other special guidance and notes based on common misuses or confusion surrounding the V codes. The Hospital V code table includes two additional columns of information alerting you to the specific V codes that are CCs and to the specific V codes that are or are not allowed to be PDX.
